Important Note:
The following are counseling services provided at FCS and the standard fee
scales. All fees are due in full at the time services are rendered unless
other payment arrangements have been made with the Director prior to treatment.
Any monies which are refundable will be returned in the form of a check from
Freedom Counseling Services, Inc. to the entity or individual that originally
provided payment. Payment must be made in the form of cash, money order,
cashier's check or company check. A personal check may be accepted only if
prior arrangements are made with the Director. All fees are on a sliding
fee scale basis. For any reduction in the required fee, arrangements must
be made with the Director at the time of the placement interview. Proof of
financial need may be required for a request in the reduction of the fee to be
considered. Some fees will not be eligible for a fee reduction based on
referral agency requirements. Drug Screens (Nine Panel) will be
administered if required by the referring agency or requested by the client.
$20 per session

This group
meets one time per week for one hour. This service is an early
intervention for the persons who are exploring the possible need for
treatment. These persons may be referred for treatment by their
family, other support group, employer or the legal system. The
participant will be assisted in recognizing the harmful consequences of
inappropriate substance abuse. Placement interview required prior to
admission to this group. (Also provided in Spanish.)

Outpatient Counseling (ASAM1 Level
l)
- $20 per session

These groups meet two times per week on an ongoing
basis. Each session will be 1½ hours in
duration. This group will address major lifestyle, attitudinal and
behavioral issues which could undermine the individual's goals of treatment.
The participants will be led to develop motivation for treatment.
He/she will also be assisted in learning to deal with life issues in a
productive manner without substance abuse. Placement interview
required prior to admission to this group. Completion of this group
requires at least thirteen consecutive weeks of attendance (exception may
only be granted by the Director in writing).

Intensive Outpatient Counseling Groups (ASAM1
Level ll.1)
- $40 per session

These groups meet three times per week on an ongoing
basis. Each session will be three hours in duration. This
service is for the persons who have already had a number of life events
occur that give evidence of an AOD abuse or dependency. Placement
interview required prior to admission to this group. Completion of
this group requires at least nine consecutive weeks of attendance (exception
may only be granted by the Director in writing).

Christ-Centered Recovery Residences (ASAM1 Level lll.1)
- $170 per week

A six to twelve month structured living arrangement
that provides a positive recovery environment. Residents
must maintain a recovery lifestyle which includes: participation
in the counseling services determined to meet his need, 12-step meetings and
Celebrate Recovery meetings, nightly Bible Study and prayer as a group in
the residence, and church attendance in a local church. The resident
will also be required to work a 40 hour per week day-time job. Nine
panel drug screens will be administered on a regular basis. The cost
includes the counseling service, housing, and food. (At this time,
this service is only available for men.)

Relapse
Prevention Therapy Groups
- $30 per session

These groups meet three times per week for nine
weeks. Each session will be 1½ hours in
duration. This service is for the individual who has had a period of
recovery but then has relapsed and either used, drank or had a "dry" or
"clean" relapse. These are closed ended groups and require attendance
in all groups for successful completion. Placement interview required
prior to admission to this group.

Individual AOD Counseling
- $45 per session

Each session is by appointment only and is 50
minutes in duration.

Family Groups
- $10 per session

These groups will meet once a week for six weeks.
These are closed ended groups requiring prior admission to this group.

Family Education Seminar
- Free

These events will take place periodically throughout
the year on an "as needed" basis. Families will learn how to help but
not enable the addicted individuals. These presentations will take
place not only in the center, but in churches, schools, and other public
venues as well. (Also provided in Spanish.)

DUI Multiple Offender Groups (Pending DHR
Approval)
- $40 per session

This service (if approved by the Georgia Department
of Human Resources) will meet the requirements for individuals referred for
such treatment in order to secure a valid Georgia Driver's License.
The required attendance of three hours per week for 17 consecutive weeks
must be met to successfully complete this program. (Will also be
provided in Spanish.)
